چکیده

INTRODUCTION Reports describing respiratory function of patients after conventional or minimally invasive cardiac surgery are infrequent. AIM To compare pulmonary functional status after conventional (AVR) and after minimally invasive, through right anterior minithoracotomy, aortic valve replacement (RT-AVR). MATERIAL AND METHODS This was an observational analysis of 212 patients scheduled for RT-AVR and 212 for AVR between January 2011 and December 2014 selected using propensity score matching. Respiratory function based on spirometry examinations is presented. RESULTS Hospital mortality was 1.4% in RT-AVR and 1.9% in AVR (p = 0.777). Predicted mortality (EuroSCORE II) was 3.2 ±1.1% in RT-AVR and 3.1 ±1.6% in AVR (p = 0.298). Mechanical ventilation time in intensive care unit (ICU) was 7.3 ±3.9 h for RT-AVR and 9.6 ±5.5 h for AVR patients (p < 0.001). Seven days and 1 month after surgery, the reduction of spirometry functional tests was greater in the AVR group than in the RT-AVR group (p < 0.001). Three months after surgery, all spirometry parameters were still reduced and had not returned to preoperative values in both RT-AVR and AVR groups. However, the difference in spirometry values was no longer statistically significant between RT-AVR and AVR groups. Presence of chronic obstructive pulmonary disease and conventional AVR surgical technique were associated with lower values of spirometry parameters after surgery in linear median regression. CONCLUSIONS Respiratory function based on spirometry examinations was less impaired after minimally invasive RT-AVR surgery in comparison to conventional AVR surgery through median sternotomy.

Minimally Invasive Approaches Versus Conventional Sternotomy for Aortic Valve Replacement: A Propensity Score Matching Study

BACKGROUND The aim of this study is to evaluate our institutional results of the aortic valve replacement through minimally invasive approaches compared with conventional sternotomy. MATERIALS AND METHODS From August 1997 to July 2010, 838 patients underwent primary isolated aortic valve replacement. Of them, 73 patients underwent surgery through minimally invasive approaches (MIAS group) whe...

Ministernotomy versus conventional sternotomy for aortic valve replacement: matched propensity score analysis of 808 patients.

OBJECTIVES The proportion of minimally invasive approaches is rising in cardiac surgery, in part driven by increasing patient demand. This study aimed to perform a risk-adjusted comparison of mortality, rate of stroke and perioperative morbidity of aortic valve replacement (AVR) conducted through either partial mini-sternotomy or conventional sternotomy. METHODS Between July 2009 and July 201...

Transapical aortic valve implantation in 100 consecutive patients: comparison to propensity-matched conventional aortic valve replacement.

AIMS To evaluate the outcome of transapical aortic valve implantation (TA-AVI) in comparison to conventional surgery. METHODS AND RESULTS One hundred consecutive high-risk patients with symptomatic aortic valve stenosis received TA-AVI using the Edwards SAPIEN pericardial xenograft between February 2006 and January 2008. Patient age was 82.7 +/- 5 years, 77 were females, logistic EuroSCORE pr...

Minimally invasive versus conventional mitral valve surgery: a propensity-matched comparison.

OBJECTIVE Less invasive approaches to mitral valve surgery are increasingly used for improved cosmesis; however, few studies have investigated their effect on outcome. We sought to compare these minimally invasive approaches fairly with conventional full sternotomy by using propensity-matching methods. METHODS From January 1995 to January 2004, 2124 patients underwent isolated mitral valve su...

Single Centre Experience with Minimally Invasive Aortic Valve Replacement versus Conventional Full Sternotomy Approach - A Propensity Match Analysis

Method Between December 2010 and March 2012 627 patients underwent isolated AVR were retrospectively included in two groups: 599 patients underwent FS-AVR sternotomy (Group A), while 28 underwent minimally invasive procedure (Group B). Mini-AVR was performed through a 6 cm upper midline incision with reverse ‘J’ manubriotomy carried into the right third intercostal space. Venous drainage for ca...
